i965: Updated fixed-point sizes in Ivybridge SAMPLER_STATE.

Texture LOD Bias is now S4.8 instead of S4.6;
Min LOD, and Max LOD are now U4.8 instead of U4.6.

Fixes piglit test tex-miplevel-selection.
