(main): Handle -U before -D (and -A).

(rescan): If traditional, recognize # only at bol.

(main) [VMS]: Use .obj, not .o, in dependencies output.

(do_include): Delete unreachable goto after '"' case.

(do_xifdef): Skip all kinds of whitespace, in bof test.
