regrename.c: Rewrite to handle multi-register modes and cond_exec instructions.

        * regrename.c: Rewrite to handle multi-register modes and
        cond_exec instructions.
        * Makefile.in (regrename.o): Update dependancies.
        * recog.h (struct operand_alternative): Add is_address.
        * recog.c (preprocess_constraints) [case 'p']: Set it.
