Change dwarf2_per_objfile::quick_file_names_table to htab_up

Change dwarf2_per_objfile::quick_file_names_table to htab_up

This changes dwarf2_per_objfile::quick_file_names_table to be an
htab_up.  This just removes a bit of manual management.
