cppopts.texi: Add conflicting option note to -dM.

    * doc/cppopts.texi: Add conflicting option note to -dM.
    * doc/invoke.texi: Add note about possible conflicts with
    -E for -dCHARS and note that -dM will not produce
    any results if there is no machine dependent reorg.
