cppspec.c: Treat two non-option arguments as input and output file.

* cppspec.c: Treat two non-option arguments as input and
output file.  Three or more non-option args is an error.
Clean up.
* gcc.c (default_compilers): Pass -$ to the preprocessor.
* objc/lang-specs.h: Likewise.
