instantiate5.C: New test.

* g++.old-deja/g++.pt/instantiate5.C: New test.  `global
constructors' name is not unique
* g++.old-deja/g++.pt/instantiate5.cc: ditto
* g++.old-deja/g++.pt/instantiate5-main.cc: ditto
