Use DISABLE_COPY_AND_ASSIGN

Use DISABLE_COPY_AND_ASSIGN

We have many classes that copy cotr and assignment operator are deleted,
so this patch replaces these existing mechanical code with macro
DISABLE_COPY_AND_ASSIGN.
