glapi: add ARB_gpu_shader_fp64 (v2)

Just add the xml file covering this extension,
and dummy interface files in mesa, and fix up
sanity tests.

v2:
Enable ProgramUniform*d* from ARB_separate_shader_objects (Ian)
use 40 instead of 43 for dispatch_sanity.cpp (Chris)
uncomment PU sanity tests.
